The following forms and instructions are for families who have already been accepted to the school. If you are interested in registration, please see our admissions page for information and a registration/waitlist form.

Please complete the following forms and drop them off at Laurelwood Preschool or mail them to:

All forms and April 2016 Tuition must be delivered or postmarked by May 1, 2015 before your child's position will be secured.
May & April 2016 tuition will be refunded ONLY if the registrar is notified BEFORE July 1, 2015 of your plans to withdraw. No Exceptions.
Instructions
- Download forms 1-8.
- Print and complete all requested information on each form.
- Collect items 8, 10, and 11 from your health care provider.
- Write your Student's and Teacher's Name on an envelope.
- Put completed forms 1-11 and April 2014 Tuition in the envelope.
- Either mail or drop off by May 15, 2013 to secure your child's position.

Parents are required to notify the registrar and modify these cards in the classroom immediately each time there is a change in any information for the cards.

Parents are required to notify the registrar and modify these cards in the classroom immediately each time there is a change in any information for the cards.
8. Physician's Report (Applies to new enrollees only)
Please download, complete and obtain your doctor's signature. All children must have a physical examination completed by his/her doctor within the one year prior to enrollment in the school and be determined to be a "healthy child”. It becomes very difficult to get appointments as spring progresses so we highly recommend you make arrangements for this now.

9.Birth Certificate (Applies to new enrollees only)
Please provide a photocopy of the child's birth certificate.
10.Immunization History (Applies to new enrollees only)
Please provide a photocopy (keep your original) of your child's immunization record. It must include the following minimum required dosages: DTP - 4 doses, Polio - 3 doses, Measles, mumps, rubella - 1 dose, Varicella – 1 dose, Hib Meningitis - 1 dose, Hepatitis B - 3-dose series.
11. Negative TB Test (Applies to new enrollees only)
Please provide a copy of a negative TB test for every adult (parents, grandparent, babysitter, etc) who will be working in the classroom. This test must have been given and read in the one year prior to the enrolling child's association with Laurelwood Preschool. TB tests are available at County Health Departments throughout the area for a nominal fee - call 792-5200 or through your local doctor.
